The 3-Year Certification Cycle

Year 0 — Certification

Gap analysis, documentation, implementation, internal audit, Stage 1 document review by CB, Stage 2 on-site audit. Certificate issued for 3 years on successful completion.

Year 1 — Surveillance Audit 1

First annual surveillance audit by the Certification Body. Verifies continued compliance with the standard. Non-conformities from the initial audit should be closed.

Year 2 — Surveillance Audit 2

Second annual surveillance audit. Assesses whether the management system is improving and addressing non-conformities identified in Year 1.

Year 3 — Recertification Audit

Full recertification audit at the end of the 3-year cycle. Successful recertification extends the certificate for another 3 years. Failure to recertify results in suspension of the certificate.

Understanding Audit Findings

Major Non-Conformity:

A systematic failure to meet a clause of the ISO standard. Must be corrected with root cause analysis before the certificate can be issued. Serious business risk if unresolved.

Minor Non-Conformity:

An isolated lapse or deviation from the standard. A corrective action plan must be submitted. The certificate can be issued, but closure must be confirmed at the first surveillance audit.

Observation / Opportunity for Improvement:

Not a conformity issue but the CB recommends improvement. No formal corrective action required, but best practice to address before the next surveillance audit.
